Pilots and flight attendants are drawing fresh attention after a Harvard analysis tied their jobs to higher rates of certain cancers. The study looked at millions of death certificates and found radiation-related risks that stood out sharply compared with more than 500 other occupations.
The research centered on cosmic radiation, which builds as altitude climbs and the shielding from the atmosphere thins out. That kind of exposure does not hit every worker the same way, and people who spend years in the cockpit or cabin appear to carry a heavier burden than the average traveler.
Researchers used data from the National Vital Statistics System and reviewed nearly 13 million U.S. death certificates from 2020 through 2024. Within that massive pool, they identified about 14,000 pilots and 7,000 flight attendants, then compared their mortality patterns with those from other jobs.
The cancers reviewed included breast cancer, multiple myeloma, leukemia, lymphoma, central nervous system cancers, thyroid cancer, prostate cancer, melanoma, and non-melanoma skin cancers. Lung cancer was left out so smoking would not blur the picture, a move that helped keep the focus on radiation-linked outcomes.
What stood out most was the pattern among airline crew members. Pilots and flight attendants showed statistically significant higher mortality from breast cancer, central nervous system cancers, prostate cancer, and melanoma, while pilots also had a higher risk of leukemia.
Among flight attendant deaths, 6.9% were tied to radiation-related cancers. For pilots, the figure was 6.7%, and both numbers were higher than those seen in any other occupation in the study.
That does not mean every person in those roles faces the same danger, and the researchers were careful about that point. Still, the overall trend was strong enough to raise questions about whether airline workers should be screened more aggressively or earlier in life.
Experts have long known that time in the air brings radiation exposure, but this study gives the concern a sharper edge. Airline workers are estimated to receive about 3 to 6 millisieverts of cosmic radiation each year, compared with about 0.4 millisieverts for someone taking 10 round-trip cross-country flights.
That gap means crew members can absorb roughly eight to 15 times the annual exposure of a frequent passenger making those same trips. Over a long career, that adds up, and the study suggests cumulative exposure may be part of the reason these jobs stand out.
The findings also leave room for other influences, including lifestyle and workplace factors that were not fully captured in the data. Even so, the comparison still pointed back to a clear occupational pattern that deserves more scrutiny.
For people who fly for work every so often or rack up miles on vacation, the risk picture looks different. The study’s concern is aimed more at those who live in the sky day after day, not the typical traveler who boards a plane a few times a year.
That distinction matters, because it keeps the conversation grounded instead of alarmist. The real question now is how airlines, doctors, and regulators respond to a job where the view from 35,000 feet may come with a quiet medical cost.
