Alcohol-related cancer deaths have climbed sharply over the past three decades, and the newest research points to a pattern that is especially alarming in adults under 55. The study links rising death counts to a wider spread of cancer types, with younger men showing a surprising shift toward colorectal cancer as a leading cause. Researchers say the message is simple but hard to ignore: alcohol is not just a liver issue, and the risk reaches farther than many people realize.
The analysis, led by researchers at the Sylvester Comprehensive Cancer Center at the University of Miami Miller School of Medicine, found that annual alcohol-attributable cancer deaths more than doubled from 11,361 in 1990 to 23,126 in 2023. The findings were published in The Lancet Regional Health – Americas and shared at the American Society of Clinical Oncology Annual Meeting in Chicago. To track the trend, the team reviewed more than 30 years of data from the Global Burden of Disease study.
That long view matters because alcohol has already been labeled a Group 1 carcinogen by the International Agency for Research on Cancer. Even so, the public still tends to underestimate how closely drinking and cancer are connected. Chinmay Jani, M.D., the study’s corresponding author and chief fellow in hematology and oncology at Sylvester, said the most surprising part was how many cancer types showed up in the picture.
“The most surprising finding was the breadth of alcohol’s potential impact across cancer types,” Jani said. “Although its relationship with liver cancer is widely recognized, its contribution to cancers such as colorectal, esophageal, breast, pancreatic and prostate cancer is less understood by the public.” That kind of spread is exactly what makes the issue so tricky, because the danger does not stay locked inside one diagnosis or one age bracket.
The burden was heaviest among older adults and men overall, but the breakdown by age and sex revealed some stark differences. Among older men, liver cancer led the toll from alcohol, with esophageal and colorectal cancers close behind. For women, breast cancer was the leading cause of alcohol-related cancer deaths, showing that the pattern changes depending on who is being studied.
The younger group is where the findings really stand out. Among adults ages 20 to 54, colorectal cancer became the leading cause of alcohol-attributable cancer deaths in men and the second leading cause in women, overtaking liver cancer in both groups. That shift is especially unsettling because colorectal cancer is often associated with older adults, yet the data suggest alcohol may be helping drive risk much earlier in life.
Researchers also stressed that alcohol’s cancer connection is still underappreciated compared with other known risks, especially tobacco use. Awareness matters here, because many people still think of drinking as a social habit with only short-term consequences. The study suggests that the long-term damage can be far more serious, and it can show up in organs and cancers people do not immediately connect with alcohol.
One of the more sobering takeaways is that no minimum safe level of alcohol has been established. That does not mean every drink is identical in effect for every person, but it does mean the old comfort of a hard cutoff is not there. From a public health standpoint, researchers say drinking less could help lower long-term cancer risk, especially when the goal is to reduce preventable harm.
At the same time, the authors were careful about the limits of the data. The study used existing national records rather than following individuals over time, so it could not track each person’s exact drinking pattern or account for every lifestyle factor. That means the results show a strong association between alcohol use and cancer deaths, but they do not prove direct causation or set a precise “safe” drinking limit.
Gilberto Lopes, M.D., senior author and chief of the Division of Medical Oncology at the Miller School, said the bigger lesson is that alcohol-related cancer risk cuts across diseases and demographics. “What this study makes clear is that alcohol-related cancer risk is not limited to one disease or one group of people,” Lopes said. “As physicians and cancer researchers, we have an opportunity to help patients understand that alcohol is a modifiable risk factor and that even small, informed changes can be part of a broader strategy to reduce cancer risk in our community.”
That framing puts the focus where it belongs, on choices that can be changed. As more adults see how drinking fits into the cancer picture, the conversation is likely to get louder, especially for younger people who may not think they are anywhere near the risk zone. The numbers are already there, and they are hard to shrug off.
