Hantavirus Outbreak On Antarctic Cruise Prompts Passenger Tracing

The MV Hondius voyage that left Ushuaia turned into an international public health incident after multiple passengers fell ill and several died, sparking an investigation into a hantavirus outbreak linked to the Andes virus and triggering cross-border evacuations and contact tracing.

The Dutch-operated expedition ship sailed from Ushuaia on April 1 with a route that included Antarctica and remote South Atlantic islands. Early in the trip a 70-year-old Dutch man developed fever and mild gastrointestinal symptoms after touring in Argentina and Chile. His condition worsened to respiratory distress and he died aboard on April 11, a grim first sign that something serious was unfolding.

On April 15 six passengers boarded at Tristan da Cunha while the deceased passenger’s body remained on board, and the cruise kept moving toward other stops. The vessel reached St. Helena on April 24 where the man’s body was taken ashore and a number of passengers disembarked. Among those who left the ship was the man’s wife, who was already feeling unwell.

On April 25 the woman, symptomatic, boarded a commercial flight from St. Helena to South Africa with many other travelers and crew. She collapsed at a South African airport on April 26 and later died, adding a second fatality linked to the voyage. Authorities began to piece together a pattern of severe respiratory illness among people connected to the ship.

On April 27 a British passenger fell ill after the ship left St. Helena and was evacuated to South Africa for intensive care with high fever and signs of pneumonia. Another passenger from Germany became sick on April 28 as the Hondius approached waters off West Africa. By May 2 the German passenger had died on board, bringing the confirmed death toll to three.

Laboratories confirmed hantavirus in the British patient, and South African investigators ran posthumous tests that identified hantavirus in the Dutch woman who collapsed in the airport. The World Health Organization stepped in on May 3 to investigate a suspected outbreak tied to the cruise. On May 4 the WHO officially classified the situation as an outbreak after confirmation of multiple infections.

The ship requested permission to disembark sick passengers in Cape Verde on May 5 but was denied, creating a tense standoff as onboard care teams and Cape Verde health workers tried to manage cases without allowing people ashore. Two crew members, including the ship’s doctor, fell seriously ill and others were monitored for symptoms. International pressure built to find a port that would accept the vessel and its patients for proper medical evacuation.

On May 6 three seriously ill people, two of whom tested positive for hantavirus, were flown to specialized hospitals in Europe after arrangements were made to receive them. The Hondius then headed toward Spain’s Canary Islands where authorities prepared screening, quarantine and repatriation plans. Switzerland later reported another confirmed case in a man who had disembarked earlier in St. Helena, increasing the number of known infections.

Health officials determined the circulating pathogen was the Andes virus strain, notable because it is the hantavirus species thought capable of human-to-human transmission in rare circumstances. That finding altered the public health response and raised alarm in countries with passengers who had already returned home. Several nations began isolating former travelers and launching contact tracing to identify anyone who might have been exposed.

By May 7 authorities in Switzerland, Britain, the Netherlands, France, Singapore, South Africa and other countries were tracing contacts and isolating passengers who had left the ship earlier. The Hondius arrived near Tenerife on May 8 where Spanish officials organized an evacuation and repatriation operation that included onboard screening before disembarkation. Local health services also reported a resident of Tristan da Cunha may have been exposed and was hospitalized for observation.

Officials noted possible secondary exposures beyond the ship, including two New Jersey residents who were monitored after potential contact during travel. The international response continues to focus on careful screening, quarantine where needed and close monitoring of anyone who shared the ship or later crossed paths with its passengers. The episode underlines how quickly a remote expedition can become an international health challenge when a serious pathogen is involved.
