CDC Warns Shigella Resistance Surges, Standard Treatments Fail

Shigella is making a nasty comeback in a more dangerous form, and the real problem is not just the bug itself. More samples are now showing up as extensively drug-resistant, which means the usual medicines are losing their edge and doctors are having fewer easy answers. The infection can spread through everyday contamination and during sex, which gives it more ways to move quietly through communities.

CDC data paints a sharp picture of the shift. Out of nearly 17,000 Shigella samples reviewed from 2011 through 2023, 8.5% of those with resistance data were classified as extensively drug-resistant in 2023, while none were in the earlier years of the study. That does not point to a massive explosion in total infections, but it does show the bacteria are adapting faster than the treatment playbook.

That distinction matters. The case count can look stable while the medicine cabinet gets weaker, and that is exactly the kind of change public health experts hate to see. When antibiotics stop working, an infection that used to be manageable can turn into a longer, messier, and more expensive problem.

Shigella spreads when tiny traces of stool make their way into someone’s mouth, whether through contaminated food or water, unwashed hands, or sexual contact. It is not usually labeled a classic sexually transmitted infection, but that does not mean sex cannot help pass it along. In that setting, close contact and exposure to fecal matter can make transmission surprisingly easy.

The CDC report also showed who is showing up most often in the resistant cases. Most patients were adult men, and a large share of those with known HIV status were living with HIV. People experiencing homelessness and those with weakened immune systems may also be at higher risk, which makes prevention and early care even more important.

Children under 5 are still among the groups most likely to catch Shigella, especially in child care and school settings. Earlier outbreaks in the United States often hit children and involved strains that were still sensitive to treatment, but the newer resistant strains raise the stakes for everyone. What used to be a routine stomach bug can now trigger real concern if it lingers or gets severe.

Symptoms can include diarrhea, fever, and abdominal cramps, and in tougher cases people may end up hospitalized. The CDC found that more than a third of patients with hospitalization data were admitted, which is a reminder that this is not always a quick, harmless illness. Even when there were no deaths in the analyzed XDR cases, the burden on patients and doctors is obvious.

Doctors say caution matters because some common diarrhea medicines can make things worse by slowing the gut. If Shigella is suspected, medical advice should come before reaching for over-the-counter symptom relief, especially when the illness is intense or not improving. Most infections clear on their own, but severe cases may need targeted treatment, and the wrong move can prolong the ride.

Prevention still comes down to basics that are easy to ignore and hard to argue with. Handwashing, cleaner hygiene before and after sex, condoms or dental dams, and cleaning sex toys after use can all cut the odds of spreading the bacteria. If a partner has diarrhea, backing off from sex until well after symptoms end is the safer call, because feeling better does not always mean the infection is gone.
